Avocare is led by Dr. Khalid Shirif who is board certified in Palliative Care and the interdisciplinary team spcialized in managing chronic health conditions includes doctors, nurse practictioners, nurses, social workers and other clinical staff will work with you and other doctors to develop a care plan and maintain close contact to better manage chronic condition.  We explain medications and disease progression, coordinate care with specialists, refer to community resources and assist with medical directives.
